Fill out the application by clicking the link at the very bottom of this page. When filling out you will notice the first section is for the prospective CIT to fill out with a Parent/Guardian signature, while the second section is for the Parent/Guardian to complete.

A current physical and immunization record are both required. Please email, fax, drop at our mailbox, or mail the forms in.
As of January 2022 covid vaccination cards are not required to be submitted. Our information is below:
